What is Hypospadias?

Hypospadias is a congenital condition in males where the opening of the urethra (urethral meatus) is not located at the tip of the penis (glans), but on the underside of the penis, scrotum, or perineum. Commonly referred to as 'Prophet's Circumcision' in some cultures due to the incomplete formation of the foreskin, it requires precise surgical correction.

Causes and Symptoms of Hypospadias

During fetal development (typically weeks 8-14 of gestation), incomplete closure of the urethral folds leads to hypospadias. Hormonal imbalances, genetic predisposition, and environmental factors play a role. Common clinical features include:

  • Displacement of the urethral opening along the ventral surface
  • Downward spraying or deflection of the urinary stream
  • Ventral penile curvature known as chordee
  • Incomplete, hooded appearance of the prepuce (foreskin)
  • In severe cases, inability to void while standing

Classification of Hypospadias

Hypospadias is classified based on the anatomical location of the urethral meatus relative to the glans:

  • Distal (Anterior) Hypospadias: The opening is near the glans or coronal sulcus. Represents ~65-70% of cases with high surgical success.
  • Midshaft (Middle) Hypospadias: The meatus is located along the penile shaft (~10-15% of cases).
  • Proximal (Posterior) Hypospadias: The meatus is located near the scrotum or perineum (~15-20% of cases), often associated with severe chordee.

Surgical Techniques for Hypospadias Repair

The treatment for hypospadias is surgical reconstruction (urethroplasty). The main goals are to construct a functional neo-urethra, straighten any penile curvature (chordee correction), and achieve normal aesthetic appearance. Key surgical procedures include:

  • TIP (Tubularized Incised Plate / Snodgrass Repair): The most widely utilized single-stage procedure involving midline incising of the urethral plate to construct a tubular neo-urethra.
  • Mathieu Flap Procedure: Utilizes a meatal-based skin flap for distal defects.
  • Two-Stage Grafting: Employed in severe proximal cases where buccal mucosa or preputial grafts are used in two separate surgical phases.

Optimal Age for Hypospadias Surgery

Pediatric urology guidelines recommend performing hypospadias repair between 6 to 18 months of age. Surgery during this window offers rapid tissue healing, reduces psychological trauma associated with genital awareness, and enables normal voiding before toilet training.

Critical Warning: Avoid Routine Circumcision

Infants suspected of having hypospadias must not undergo routine circumcision. The prepuce (foreskin) serves as an essential source of vascularized tissue skin flaps or grafts needed during surgical reconstruction.

Postoperative Care and Recovery

Following surgery, a small silicone urethral stent or catheter is temporarily placed for 5 to 10 days to allow the neo-urethra to heal. Post-surgical care involves prophylactic antibiotics, double-diapering technique to protect the stent, and minimizing physical trauma during recovery.
