What is a Bladder Pacemaker (Sacral Neuromodulation)?

A bladder pacemaker, clinically known as sacral neuromodulation, is a therapeutic intervention based on stimulating the nerves that control bladder function with mild electrical impulses. It is primarily indicated for refractory conditions that do not respond to conservative treatments, such as overactive bladder, urge incontinence, non-obstructive urinary retention, and fecal incontinence. The system consists of a thin lead placed near the sacral nerves at the base of the spine and a battery-powered generator.

How is Bladder Pacemaker Surgery Performed?

The treatment involves a two-stage surgical process. The first stage is a trial (evaluation) phase, and the second stage is the permanent implantation of the generator. This staged approach ensures that the patient will benefit from the therapy before committing to a permanent device.

Stage 1: The Trial Phase

Performed under local anesthesia or mild sedation, this phase involves placing a temporary lead near the sacral nerve through a small incision in the lower back. This lead is connected to an external stimulator worn on a belt. The patient keeps a bladder diary for 1 to 2 weeks. If symptoms improve by at least 50%, the patient is deemed a candidate for the permanent implant.

Stage 2: Permanent Implantation

If the trial phase is successful, the permanent neurostimulator (battery) is implanted. Typically performed under general anesthesia or local sedation, a small pocket is created under the skin in the upper buttock area. The temporary external controller is removed, and the lead is connected to the permanent generator placed in the pocket. The incision is then closed with sutures.

How Long Does a Bladder Pacemaker Battery Last?

The lifespan of a bladder pacemaker battery depends on the device type and the programmed stimulation settings. Currently, two main types of batteries are utilized:

  • Non-Rechargeable Batteries: These devices typically last between 3 to 5 years. Once depleted, the battery unit is replaced via a minor outpatient surgical procedure under local anesthesia.
  • Rechargeable Batteries: These devices, which the patient periodically recharges wirelessly, can have a functional lifespan of up to 15 years.

Postoperative Recovery and Care

Most patients are discharged on the same day or the day following the permanent implantation. During the first few weeks, patients should avoid heavy lifting and excessive bending or twisting to prevent lead displacement. Once healed, patients can resume normal daily activities. While many modern devices are MRI-conditional, patients must consult their physician to program the device appropriately before undergoing any MRI scans.
