What is Microsurgical Varicocelectomy?

Microsurgical varicocelectomy is a specialized surgical procedure performed under a high-power operating microscope to ligate enlarged veins (varicocele) in the scrotum. Varicocele is one of the most common and treatable causes of male infertility, negatively impacting sperm production, motility, and morphology by increasing testicular temperature and causing toxic accumulation. The microsurgical approach is considered the gold standard treatment, offering the highest success rates with the lowest risk of complications.

Advantages of the Microsurgical Technique

Compared to conventional open surgery or laparoscopic methods, microsurgery offers significant advantages. The surgical microscope magnifies the operating field 10 to 15 times, allowing the surgeon to clearly distinguish delicate structures. Key benefits include:

  • Preservation of Testicular Arteries: The vital arteries supplying blood to the testes are identified and preserved, preventing testicular atrophy.

  • Lowest Recurrence Rate: Because even the smallest abnormal veins are identified and ligated under magnification, the recurrence rate drops below 1%.

How is Microsurgical Varicocelectomy Performed?

The procedure is typically performed under general or regional anesthesia on an outpatient basis. The surgical steps include:

  • Incision: A small incision of approximately 2-3 cm is made in the groin area (inguinal or subinguinal).
  • Microscopic Evaluation: The spermatic cord is isolated and examined under the surgical microscope to separate all vascular structures.
  • Ligation and Division: Dilated, malfunctioning veins are carefully isolated, tied off, and divided, while the vas deferens (sperm duct) and arteries are preserved.
  • Closure: The incision is closed using absorbable sutures, leaving minimal scarring.

Postoperative Recovery and Expectations

Most patients return home on the day of the surgery. Mild pain or discomfort in the groin is normal and can be managed with standard pain relievers. Patients can typically return to light activities within 3 to 5 days, while heavy lifting and sexual activity should be avoided for 2 to 4 weeks. Improvement in sperm parameters (count, motility, and morphology) is monitored via semen analysis at 3, 6, and 9 months post-surgery.
