Robotic Surgery and Open Surgery: Basic Definitions and Approaches

Modern surgical practices have evolved from traditional methods to minimally invasive techniques with technological advancements. Open surgery is a conventional method where the surgeon directly visualizes the operative field through large incisions. Robotic surgery, on the other hand, is an advanced technique performed through millimeter-sized incisions using high-definition consoles and precise robotic arms controlled by the surgeon.

Comparison of Surgical Precision and Visualization

One of the most prominent differences between the two methods is the quality of visualization and maneuverability during the operation. Robotic surgical systems provide the surgeon with a high-definition (HD), three-dimensional (3D) view magnified up to 10 to 15 times. This plays a critical role in preserving nerves and vessels, particularly in deep pelvic regions. In open surgeries, the surgeon works with the naked eye and a two-dimensional depth perception.

Incision Sizes and Tissue Trauma

The physical impact of the surgical methods on tissues directly determines the recovery process:

  • Open Surgery: Large incisions, typically ranging from 10 to 25 centimeters, are made to access the surgical site, leading to more trauma in muscle and skin tissues.
  • Robotic Surgery: Access is gained through 4 or 5 small ports, usually not exceeding 8 millimeters. Since muscles are not transected, tissue damage remains minimal.

Blood Loss and Transfusion Requirements

In robotic surgery, gas pressure and the precision of micro-instruments minimize capillary bleeding. Consequently, intraoperative blood loss is extremely low, and the need for blood transfusion is rare. In open surgery, the risk of blood loss is higher due to the large incision area and vessel exposure.

Recovery Process, Hospital Stay, and Pain Management

Postoperative patient comfort and return to daily activities depend heavily on the surgical approach. Patients undergoing robotic surgery experience less pain due to small incisions and require fewer narcotic analgesics. They are typically discharged within 1 to 2 days. In open surgery, healing of large incisions, wound care, and pain management take longer, with hospital stays often extending to 5 to 7 days.

Complication and Infection Risks

Surgical site infections, hernia formation, and postoperative adhesions occur more frequently in open surgery due to the large wound surface. In robotic surgery, the risk of infection is significantly reduced as the body's internal environment has minimal contact with the external atmosphere. Additionally, the tremor-filtering technology of robotic arms minimizes the risk of injury to surrounding tissues and organs.
