The Role of Physical Activity in Passing Kidney Stones
The movement of kidney stones through the urinary tract (ureter) and their elimination from the body depend on gravity and the peristaltic contractions of the ureter. Being physically active can accelerate this process. Movement reduces the risk of the stone getting stuck, facilitating its downward progression along with urine flow.
Which Exercises Help Pass a Kidney Stone?
Not all exercises are safe or effective during this process. Low-impact activities that create vertical vibration and leverage gravity are preferred:
- Brisk Walking: Boosts blood circulation and urinary tract motility, supporting the stone's progression.
- Climbing Stairs: The vertical rhythmic vibration of the body helps prevent the stone from adhering to the ureteral wall.
- Light Jumping or Rope Skipping: Maximizes the gravitational effect, accelerating the downward movement of the stone. It should be performed cautiously based on pain levels.
- Gentle Stretching: Helps relax pelvic floor muscles and reduces spasms.
The Scientific Mechanism of Movement
Vertical vibrations during physical activity generate mechanical stimulation in the smooth muscles of the ureter. This stimulation triggers the natural contractile rhythm (peristalsis) of the urinary tract. Concurrently, increased blood flow during exercise indirectly supports renal filtration, increasing urine volume and fluid pressure to push the stone.
When to Rest: Contraindications
Although movement is beneficial, exercising under certain conditions can be harmful. Physical activity should be stopped immediately in the following cases:
- Severe Renal Colic (Intense Pain): Straining the body during acute pain crises can worsen spasms. Rest and medical therapy are the priorities here.
- Nausea and Vomiting: Exercise should be avoided when dehydration risks are high due to vomiting.
- Macroscopic Hematuria (Visible Blood in Urine): In case of heavy bleeding, movement should be restricted, and a physician should be consulted.
- Fever and Signs of Infection: Exercise is strictly contraindicated if an infection develops due to urinary tract obstruction.
The Synergy Between Hydration and Movement
The success of exercise in passing stones is directly related to hydration. Exercising with insufficient fluid intake causes water loss through sweat, reducing urine output. This can cause the stone to become more tightly lodged. Therefore, at least 1-2 glasses of water must be consumed every hour while active.
Medical Evaluation and Monitoring
Stones larger than 5-6 millimeters are highly unlikely to pass through movement and hydration alone. Large stones can obstruct the urinary tract, leading to kidney damage. Therefore, uncontrolled exercise regimens should not be initiated before determining the size and location of the stone via ultrasonography or CT scan.
