What is Varicocele?
Varicocele is the abnormal dilation and enlargement of the pampiniform plexus, a network of veins within the scrotum that drains blood from the testicles. Similar to varicose veins occurring in the legs, varicoceles can develop at any age from puberty onward. It represents the most common correctable cause of male factor infertility, impairing spermatogenesis and testicular function.
Causes and Pathophysiology
Testicular veins contain one-way valves that direct blood flow back toward the heart. Incompetence or structural failure of these venous valves leads to retrograde blood flow and venous stasis, resulting in increased intravascular pressure and vein enlargement.
Approximately 85-90% of varicoceles occur on the left side due to anatomical features: the left internal spermatic vein drains into the left renal vein at a right angle, creating higher hydrostatic pressure compared to the right spermatic vein, which drains directly into the inferior vena cava.
Symptoms of Varicocele
While many varicoceles are asymptomatic and discovered incidentally, progressive cases present with characteristic clinical manifestations:
- Dull Scrotal Pain: A persistent aching sensation that worsens during prolonged standing, heavy exercise, or physical strain, and subsides when lying down.
- Visible or Palpable Mass: A sensation described as a 'bag of worms' inside the scrotum.
- Testicular Atrophy: Shrinkage or loss of volume in the affected testicle due to impaired circulation and hyperthermia.
- Male Infertility: Decreased sperm count, impaired motility, and abnormal sperm morphology.
- Hormonal Dysfunction: Impairment of Leydig cell function leading to lowered testosterone levels in rare cases.
Clinical Grading of Varicocele
Varicoceles are classified into clinical grades based on physical examination findings:
- Grade 1: Palpable only during the Valsalva maneuver (bearing down).
- Grade 2: Easily palpable at rest without the need for a Valsalva maneuver, but not visible.
- Grade 3: Clearly visible through the scrotal skin prior to palpation.
- Subclinical: Not detectable via physical examination; identified only by imaging studies.
Diagnostic Methods
Accurate diagnosis requires a structured clinical assessment conducted by an urologist:
1. Physical Examination
Physical examination remains the primary diagnostic standard. It is conducted in a warm room while the patient is standing, incorporating the Valsalva maneuver to evaluate venous reflux and vein dilation.
2. Scrotal Color Doppler Ultrasound
Doppler ultrasonography provides objective measurement of venous diameter and blood flow direction. A venous diameter exceeding 3.0 mm along with persistent retrograde flow during Valsalva confirms the diagnosis.
3. Semen Analysis (Spermiogram)
Semen evaluation is essential for assessing testicular function in adult patients. At least two separate semen analyses are recommended to evaluate count, motility, and morphological integrity.
